Abstract

Red blood cell concentrates without a buffy coat (RBCs) are a blood component frequently used in transfusion support, intended to restore oxygen-carrying capacity in patients with acute or chronic anemia, hemorthage, trauma, and various hematological diseases that compromise adequate tissue oxygenation. However, erythrocytes can exhibit immunohematological alterations associated with the presence of immunoglobulins or complement fractions attached to their membrane, a condition known as erythrocyte sensitization. This condition acquires clinical importance due to its ability to reduce post-transfusion survival of the erythrocyte, promote extravascular hemolysis and generate an increase in hemoglobin of less than 1 g/dL per unit transfused during the post-transfusion evaluation, and interfere with serological compatibility, mainly in the major crossmatching test in the anti-human globulin phase, in addition to hindering the interpretation of complementary studies such as self-control, CD, elution and antibody identification. This study aimed to determine the presence of sensitized erythrocytes in CGRSCLp stored in the blood bank of Latacunga General Hospital during the period of February— June 2026. An observational, descriptive, and cross-sectional study with a mixedmethods approach was conducted using a sample of 252 units. Immunohematological analysis was performed using the CD test with gel microcolumn technique and LISS/Coombs cards, under standardized laboratory conditions. The results were classified as positive or negative, with agglutination intensity ranging from Oto4+. The analysis showed 251 non-reactive units (99.6%) and 1 reactive unit (0.4%). The positive unit exhibited weak agglutination (1+), consistent with low-intensity erythrocyte sensitization. Statistical analysis using Pearson's chi-squared test and Fisher's exact test yielded a p-value of 0.409, indicating no statistically significant association between CD positivity and blood component storage time. The detection of the unit with a positive CD test is a relevant immunohematological finding, demonstrating that an apparently suitable red blood cell concentrate (RBC) may contain sensitized erythrocytes before clinical use. This result supports the application of the CD test as an internal quality control measure prior to transfusion release, optimizing the selection of compatible blood components and protecting the safety of the recipient patient.

Los concentrados de glóbulos rojos sin capa leucoplaquetaria (CGRSCLp) representan un hemocomponente de uso frecuente en el soporte transfusional, destinada a restablecer la capacidad de transporte de oxígeno en pacientes con anemia aguda o crónica, hemorragias, traumatismos y diversas enfermedades hematológicas que comprometen la adecuada oxigenación tisular. Sin embargo, los eritrocitos pueden presentar alteraciones inmunohematológicas asociadas a la presencia de inmunoglobulinas o fracciones del complemento adheridas a su membrana, siendo conocidas como sensibilización eritrocitaria. Esta condición adquiere importancia clínica debido a su capacidad de reducir la supervivencia postransfusional del eritrocito, favorecer hemólisis extravascular y generar un incremento de hemoglobina menor a 1 g/dL por unidad transfundida durante la evaluación posterior a la transfusión, e interferir en la compatibilidad serológica, principalmente en la prueba de compatibilidad cruzada mayor en fase de antiglobulina humana, además de dificultar la interpretación de estudios complementarios como autocontrol, CD, elución e identificación de anticuerpos. El presente estudio tuvo como objetivo determinar la presencia de eritrocitos sensibilizados en CGRSCLp almacenados en la hemoteca del Hospital General Latacunga durante el período febrero — junio de 2026. Se desarrolló una investigación observacional, descriptiva y transversal, con enfoque mixto, con una muestra de 252 unidades. El análisis inmunohematológico se realizó mediante la prueba de CD por técnica de microcolumnas en gel con tarjetas LISS/Coombs, bajo condiciones estandarizadas de laboratorio. Los resultados fueron clasificados en positivos y negativos, con la clasificación de la intensidad de aglutinación de 0 a 4+. El análisis realizado mostró que 251 unidades no reactivas, correspondientes al 99,6%, y 1 unidad positiva, equivalente al 0,4%. La unidad positiva presentó aglutinación débil de 1+, compatible con sensibilización eritrocitaria de baja intensidad. El análisis estadístico mediante Chi — cuadrado de Pearson y prueba exacta de Fisher obtuvo un valor de p = 0,409, sin asociación estadísticamente significativa entre la positividad del CDy el tiempo de almacenamiento del hemocomponente. La detección de la unidad con CD positivo constituye un hallazgo inmunohematológico relevante, al demostrar que un CGRSCIp aparentemente apto puede presentar eritrocitos sensibilizados antes de su uso clínico. Este resultado respalda la aplicación del CD como control de calidad interno previo a la liberación transfusional, optimizando la selección de hemocomponentes compatibles y protegiendo la seguridad del paciente receptor.
