    Manual de estimulación temprana para el desarrollo del niño prematuro en el servicio de consulta externa

    Currently, the survival of premature babies has increased considerably, and although most of them grow normally; some have permanent sequelae that can be serious; Among the most common are alterations in neurodevelopment, which can be addressed through early stimulation techniques and activities and improve the quality of life of premature infants. In this way, the objective of this study was to design an early stimulation manual for the development of premature children in the outpatient service; Through a longitudinal investigation of 17 premature babies with corrected age referred to the External Consultation for Early Stimulation at the Hospital General Latacunga 4 of 3 months, 4 of 2 months and 9 of 1 month; to whom an initial evaluation was applied to determine their deficiencies in development through the Denver II Test and define the activities of the manual; In addition, a systematic review of scientific information was carried out that contributed to the foundation of the manual; Subsequently, a review of the manual was carried out through expert judgment and the calculation of the CVC, to finally measure the differences between the measures before and after the proposed intervention. The results showed significant differences in motor areas (fine and gross) for the three groups of children, while in the personal-social and language area there were no significant differences. Thus, it can be concluded that early stimulation in a programmed manner can contribute to the adequate development of premature children, especially favoring their fine and gross motor skills.

    La estimulación musical en el desarrollo de niños prematuros

    Musical stimulation is a tool that helps to develop capacities and skills especially in the premature population, when studied from the neurological perspective, it is a strategy that stimulates and integrates neuronal pathways to create connections through musical stimuli acting on the organization of behavior, behavioral change and learning. The main objective of the research is to determine the development of premature children and its relationship with prenatal and natal risk factors and being a study with a quantitative, prospective analytical approach, it was given from an initial evaluation carried out through the Denver II test whose results dictated a presumptive diagnosis Abnormal with 53.6% and a presumptive diagnosis with 6.7%. By means of this result, the Musical Stimulation Guide was created, which was delivered and should be applied by the representatives of the 30 premature children from 0 to 2 years old of the Early Attention Center of the Technical University of Ambato who participated in the research project. This research could not be expanded because of the sanitary emergency due to Covit-19 through decree 1074 issued by the President of the Republic, Mr. Lenin Moreno.

    The present investigation was developed on the theme "Early Stimulation in the premature newborn in the hospital setting" theme whose general objective was to determine the impact of early stimulation in premature newborns of the neonatology services of the Provincial General Latacunga Hospital and as specific objectives To investigate the techniques of early stimulation applied in the hospital environment, to evaluate the development of premature newborns in the intensive care area of neonatology and to describe the evolution of premature newborns of the neonatology services receiving early stimulation. We worked with a population of 50 live preterm newborns, this research has a qualitative-quantitative approach. For the collection of information, the technique of observation, data collection and as instrument was used the Myla assessment scale, to determine the degree of evolution of development of children admitted versus discharge, obtaining as a result that Stimulation if it influences the development of premature infants in the General Provincial Hospital Latacunga.

    “Rol de enfermería en la vinculación de los padres en el cuidado y adaptación del prematuro en el hospital general Latacunga en el área de neonatología.”

    The general objective of this research was to identify the role of nursing personnel in achieving parental involvement in the care and adaptation of the premature infant in the neonatology service of the General Hospital Latacunga. This research is quantitative, descriptive, field and cross-sectional in which the population to study was 12 graduates in nursing who work in the area of neonatology and 11 fathers and mothers of hospitalized preterm infants. 100% of the population was taken according to the inclusion and exclusion criteria. Two instruments were used to obtain the data, the first a perception survey prepared by Jahuancama and Espinoza in 2017 with the 0.94 of reliability directed to parents that consists of 20 items in relation to the perspective on the treatment provided, information and their participation in the care of their children, the second instrument was an observation guide for nursing personnel based on nursing interventions (NIC). The results showed that 92% of the nursing staff encourages the participation of parents in the care of premature babies. In relation to the perception of parents about their inclusion in the care of their child 55% sometimes refer to participate and with respect to the relationship of parents with the nursing staff most of the respondents indicate that sometimes the staff Nursing provides a kind and supportive service to parents.

    “Paraparesia espástica por alteración vascular espinal”

    The development of the present clinical case is titled: "SPASTIC PARAPARESIA BY SPINAL VASCULAR ALTERATION" the same one that refers to a patient of four years nine months of age, female sex, settled in the city of Quito. Among his personal pathological antecedents we found that he was born prematurely at thirty-five weeks of gestation due to acute fetal distress. The initial evaluation shows that the infant nation within normal ranges and was admitted to the term crib with a diagnosis of enterocolitis. The patient was referred several times to the hospitalization area. Up to six months the child has a seemingly normal development as time goes by they presented different areas of delay. At three years six months the patient is evaluated again presenting several cerebellar alterations. Different medical diagnoses were issued, finally after completing several complementary examinations, it was concluded that the infant has spastic cerebral palsy due to hyperbilirubinemia. Currently the patient goes to the physiotherapy service where they treat their physical deficiencies

    Polyhydramnios is the excessive increase of amniotic fluid 1500-2000 ml or better define distwice normal for gestational age. The following case is a patient female 17 years of age with gynecological and obstetric history of menarche at age 11, irregular menstrual cycles 1time / month for 5 days IVSA 15 years, CS # 3 diseases not sexually transmitted, paptes , current gestas 1 abortions 0, with the date of the last menstrual period 20-02- 2015 according toreliable patient taking a pregnancy Patient comes accompanied by hermotherto the emergency room of the Provincial Teaching Hospital Ambato referring abdominal pain more or less two hours contraction type located in lower abdomen and lumbar region radiating to more moderate removal of mucous splug and decreased fetal movements. Beingassessedbyphysicalexaminationfindingobstetricianbellypregnantuterus, cephalic single fetus, movingleft back, with a FCF of 136-140, decreased fetal movements and sporadic uterine activity, lower limb edema +. Genital region transvaginal no loss of fluid sise videnced touch 4 cm dilation and effacement of 70% with intact membranes ginecoide pelvis. Results ecosonography obstetric Doppler flow metry + PBF + 32.4 weeks of pregnancy not consistent with LMP, amniotic fluid index of 27.2, posterior placenta fundic 3.9cm thick, with grade III signs of maturity.
