Patient comes due to difficulty walking 3 months of evolution that is accompanied by edema, heat, flushing and deformation, at the level of the inner border of the left foot is observed ulceration of 3 centimeters in diameter with irregular edges, so go to the center of health of Alaquez where they do not perform any procedure, for which he performs cures of his ulcer with hot water and medicinal herbs, as there is no improvement go to this house of health and decides to enter vascular surgery At physical examination patient conscious, oriented, with stable vital signs, symmetrical thorax with preserved expandability; heart rhythmic heart sounds, lungs conserved vesicular murmur, abdomen soft, depressible, not painful to superficial and deep palpation, RHA present. Pulse pedions present. Decreased sensitivity and absent tendon reflexes. X-ray of the left foot is made in which slight arterial calcifications are observed, with multiple osteolytic lesions in the tarsal area and proximal parts of the first and second metatarsals. An echo Doppler is requested, which shows a clear decrease in the speeds in anterior tibial arteries and later. In laboratory tests shows leukocytosis, but neutrophilia plus thrombocytosis with elevated CRP, in blood chemistry: fasting glucose: 235 mg / dl, cholesterol: 256 mg / dl and triglycerides: 180 mg / dl. The first therapeutic behaviors used in this case were: empirical antibiotic therapy, a medication for primary hypercholesterolemia, general care, glycemia control, wound culture and cures twice a day of the ulcer plus immobilization of the left foot.

    “Amputación de miembro inferior a causa de osteomielitis”

    Among the diseases that account for the highest annual health burden are those due to environmental factors, such as road traffic injuries. "Every year, traffic accidents cause the death of approximately 1.25 million people worldwide, between 20 million and 50 million people suffer non-fatal injuries, and in turn a proportion of these suffer some form of disability". Ecuador is the second country in Latin American mortality due to traffic accidents according to the Global State Report on road safety at the World Health Organization (WHO). The data obtained show that a traffic accident is a cause of physical disability depending on the complexity of the injury and post-surgical complications. The present case analysis refers to an adult of 43 years, who after suffering a traffic accident, had exposed fractures of his lower limbs, which led to a complication called osteomyelitis characterized as an infectious process with progressive destruction of tissues soft and bony. Due to the difficult control of the infection, the patient visited several Hospital Medical Units inside and outside the country without obtaining a favorable evolution during 3 years, the doctors who followed the case decide that amputation is the only surgical medical procedure to curb the infection and safeguard their life, constituting in the long term a physical disability due to the member's recession. However prosthetic adaptation allows you to reintegrate into your daily activities, the proposed rehabilitative treatment is based on short, medium and long term objectives to avoid stump deformities and gait complications.

    This analysis of clinical case under the theme "Functional Recovery and Muscular middle finger, ring finger and little finger after amputation performed three years of the thumb and forefinger of his left hand without having attended rehabilitation done during this period" documents and analyzes so detailed visits by the patient from the trauma suffered in his hand by crushing in a work accident at the different nursing homes, collect diagnoses, interventions and treatments prescribed different doctors who had contact in the process recovery, identifies risk factors to which the patient is exposed after amputation these being: biological, environmental, social and related lifestyle of the patient, an analysis of the current situation of the patient that never came is done rehabilitation fruit of this is very strong scar adhesions, lack of functionality and strength in the remaining fingers and a protocol and treatment plan is proposed to improve the functionality in the remaining fingers.
