Biocompatibilidad en la osteosíntesis con placas de titanio vs acero inoxidable en fracturas en caninos

dc.descriptionLa selección del biomaterial en la osteosíntesis con placas ortopédicas representa una decisión clínica con repercusión directa sobre la biología ósea periimplante. El presente trabajo tuvo como objetivo comparar la biocompatibilidad de placas de titanio Ti-6Al-4V y acero inoxidable 316L en perros sometidos a fijación interna de fracturas, mediante un estudio observacional comparativo de tipo retrospectivo realizado en el Hospital Veterinario VeterPet de la ciudad de Quito, Ecuador. Se analizaron 22 pacientes caninos distribuidos en dos grupos de igual tamaño: Grupo Ti (titanio Ti- 6Al-4V, n = 11) y Grupo AI (acero inoxidable 316L, n = 11). A partir de radiografías postquirúrgicas y de control, se evaluaron cinco variables: grosor cortical periimplante medido con software ImageJ, prevalencia de stress-shielding según el criterio de Uhthoff et al. (1981), grado de consolidación ósea mediante la escala de Lane y Sandhu (1987), tipo morfológico de callo óseo y presencia de complicaciones postquirúrgicas mayores. El análisis estadístico incluyó la prueba t de Student para muestras independientes y la prueba de Chi-cuadrado de Pearson, con un nivel de significancia de α = 0,05. Los resultados mostraron que el Grupo Ti registró un incremento promedio del grosor cortical de +0,49 mm, mientras que el Grupo AI evidenció una pérdida neta de −0,72 mm, diferencia estadísticamente muy significativa (t = −6,91; gl = 20; p < 0,0001). La prevalencia de stress-shielding fue del 0% en el Grupo Ti frente al 63,6% en el Grupo AI. En cuanto a la consolidación ósea, el 36,4% de los pacientes del Grupo Ti alcanzó consolidación completa (score 4), frente al 9,1% del Grupo AI. No se registraron complicaciones mayores en ninguno de los dos grupos. Se concluye que las placas de titanio Ti-6Al-4V presentan un perfil de biocompatibilidad mecánica superior al del acero inoxidable 316L para la osteosíntesis en caninos, sin comprometer la seguridad ni la eficacia del proceso de consolidación.
dc.description.abstractThe selection of biomaterial in orthopedic plate osteosynthesis represents a clinical decision with direct consequences on periimplant bone biology. The aim of this study was to compare the biocompatibility of Ti-6Al-4V titanium and 316L stainless steel plates in dogs undergoing internal fracture fixation, through a retrospective observational comparative study conducted at VeterPet Veterinary Hospital in Quito, Ecuador. Twenty-two canine patients were distributed into two equal groups: the Ti Group (Ti-6Al-4V titanium, n = 11) and the SS Group (316L stainless steel, n = 11). Using postoperative and follow-up radiographs, five variables were assessed: periimplant cortical thickness measured with ImageJ software, stress-shielding prevalence according to the Uhthoff et al. (1981) criterion, bone healing grade using the Lane and Sandhu (1987) scale, morphological type of bone callus, and presence of major postoperative complications. Statistical analysis included the independent samples Student's t-test and Pearson's Chi-square test, with a significance level of α = 0.05. Results showed that the Ti Group recorded a mean cortical thickness increase of +0.49 mm, whereas the SS Group exhibited a net loss of −0.72 mm, a highly significant difference (t = −6.91; df = 20; p < 0.0001). Stress-shielding prevalence was 0% in the Ti Group compared to 63.6% in the SS Group. Regarding bone healing, 36.4% of Ti Group patients achieved complete union (score 4), versus 9.1% in the SS Group. No major complications were recorded in either group. It is concluded that Ti-6Al-4V titanium plates demonstrate a superior mechanical biocompatibility profile compared to 316L stainless steel for canine osteosynthesis, without compromising the safety or efficacy of the bone healing process.
