dc.descriptionLa evaluación de la efectividad dialítica es un elemento crítico que requiere constante vigilancia en el sistema de salud ecuatoriana para reducir la morbimortalidad en pacientes con Enfermedad Renal Crónica. Esta investigación evaluó una adecuada hemodiálisis mediante el aclaramiento de urea y creatinina séricas, y su correspondencia con el índice de adecuación dialítica Kt/V. Metodológicamente, se desarrolló un estudio descriptivo, correlacional y de corte transversal en una muestra de 86 pacientes sometidos a terapia sustitutiva estándar. Se recolectaron muestras sanguíneas pre y post-dialíticas con el fin de cuantificar analitos mediante espectrofotometría, posterior a esto los datos fueron analizados por medio del coeficiente Rho de Spearman en el software SPSS. Los resultados obtenidos demostraron una reducción estadísticamente significativa en las medias post-dialíticas de ambos marcadores. Se evidenció una correlación positiva entre el porcentaje de reducción de urea y el factor Kt/V (r_s = 0,997; p < 0,001), mientras que la reducción de creatinina reflejó una asociación moderada (r_s = 0,384; p < 0,001) debido a su lenta firma cinética. Por ende, se demostró que la depuración aislada de creatinina no es un predictor simétrico de adecuación, validando la necesidad de un monitoreo laboratorial síncrono e independiente para prevenir falsos negativos de suficiencia dialítica.
dc.description.abstractThe evaluation of dialysis effectiveness is a critical element and requires constant monitoring within the Ecuadorian healthcare system to reduce morbidity and mortality in patients with Chronic Kidney Disease. This research assessed adequate hemodialysis by measuring serum urea and creatinine clearance and its correlation with the dialysis adequacy index (Kt/V). Methodologically, a descriptive, correlational, and cross-sectional study was conducted on a sample of 86 patients undergoing standard renal replacement therapy. Pre- and post-dialysis blood samples were collected to quantify analytes using spectrophotometry. Subsequently, the data were analyzed using Spearman's Rho coefficient in SPSS software. The results showed a statistically significant reduction in the post-dialysis mean levels of both markers. A near-perfect positive correlation was observed between the percentage reduction in urea and the Kt/V factor (r_s = 0.997; p < 0.001), while the reduction in creatinine showed a moderate association (r_s = 0.384; p < 0.001) due to its slow kinetic signature. Therefore, it was shown that isolated creatinine clearance is not a symmetric predictor of adequacy, validating the need for synchronous and independent laboratory monitoring to prevent false negatives of dialysis sufficiency.
