DERMATOFITOSIS: INNOVACIÓN DIAGNOSTICA MEDIANTE TÉCNICAS MOLECULARES

Abstract

Dermatophytosis constitutes a group of superficial mycoses that affect the skin, nails, and hair, with Trichophyton rubrum being the most common species. Given the limitations of conventional methods, such as culture which requires long incubation periods and may lead to morphological misidentificationmolecular techniques have emerged as precise and rapid tools for detecting etiological agents. This documentary study, based on 24 articles selected through the PRISMA methodology, was conducted using an exhaustive search in the PubMed, BVS, and Web of Science databases, evaluating the most commonly used molecular methods such as PCR, ITS sequencing, MALDI-TOF, DendrisCHIP, DendrisKIT, ITS-RFLP, and LAMP, assessing their sensitivity, specificity, and diagnostic time. The results show that molecular techniques greatly outperform conventional ones in speed and reliability, achieving sensitivities and specificities close to 100%. Moreover, they enable the detection of mutations associated with antifungal resistance, such as in Trichophyton indotineae, and correct morphological identification errors. However, traditional techniques remain essential due to their low cost and accessibility. In conclusion, the integration of molecular and conventional methods represents the most effective diagnostic strategy, combining precision, speed, and clinical coverage in the management of dermatophytosis.

La dermatofitosis constituye un grupo de micosis superficiales que afecta piel, uñas y cabello, siendo Trichophyton rubrum la especie más común. Ante las limitaciones de los métodos convencionales como el cultivo, que presenta tiempos prolongados y posible error morfológico, las técnicas moleculares emergen como herramientas precisas y rápidas para la detección de los agentes etiológicos. Este estudio documental, basado en 24 artículos seleccionados mediante la metodología PRISMA, se realizó a partir de una búsqueda exhaustiva en las bases de datos PubMed, BVS y Web of Science, evaluando los métodos moleculares más utilizados, como PCR, secuenciación ITS, MALDI-TOF, DendrisCHIP, DendrisKIT, ITS-RFLP y LAMP, evaluando su sensibilidad, especificidad y tiempo diagnóstico. Los resultados evidencian que las técnicas moleculares superan ampliamente a las convencionales en rapidez y fiabilidad, alcanzando sensibilidades y especificidades cercanas al 100 %. Además, permiten detectar mutaciones asociadas a resistencia antifúngica, como en Trichophyton indotineae, y corregir errores de identificación morfológica. Sin embargo, las técnicas tradicionales siguen siendo esenciales por su bajo costo y accesibilidad. En conclusión, la integración de métodos moleculares con los convencionales representa la estrategia diagnóstica más efectiva, al combinar precisión, rapidez y cobertura clínica en el abordaje de la dermatofitosis.
