ALTERACIÓN DE LA CINEMÁTICA DEL PEDALEO RELACIONADO CON EL DOLOR PATELOFEMORAL EN CICLISTAS DEL TEAM THE DOOR´S

Abstract
Cycling, being a long-duration, high-intensity sport, is a beneficial activity. However, due to repetitive mechanical demands, it is associated with a high incidence of patellofemoral pain. Alterations in pedaling kinematics generate changes in joint loads, which can trigger anterior knee pain. The aim of the study was to correlate alterations in pedaling kinematics with patellofemoral pain in amateur cyclists from Team The Door's. The study is correlational in nature with a quantitative, prospective, and cross-sectional approach. It was conducted with 30 male cyclists, whose average age was 38.97 ± 13.54 years. To evaluate pedaling kinematics, Kinovea software with 2D analysis was used to obtain ankle, knee, and hip joint ranges. The Kujala questionnaire (AKPS) was used to assess patellofemoral pain. The data were analyzed statistically in an inferential and descriptive manner, using Pearson's correlation and Student's t-test, with a significance level of p < 0.05. The results showed a normal joint range in the knee, an ankle range with a reduction of (19.3° ± 6.0), and compensation in the hip with a wide range of (117.3° ± 7.7). One hundred percent of participants reported patellofemoral pain, especially when climbing stairs or keeping their knees bent. No correlations were found in the kinematic variables and the Kujala score. No statistically significant correlations were found in the kinematic variables and the Kujala score (r between -0.23 and -0.017; p >0.05), therefore identifying the absence of a direct relationship. In conclusion, no significant relationship was found between patellofemoral pain and pedaling kinematics.

El ciclismo, al ser un deporte de larga duración y alta intensidad es una actividad beneficiosa, sin embargo, debido a las demandas mecánicas repetitivas se asocia con una alta incidencia del dolor patelofemoral. La alteración de la cinemática del pedaleo genera cambios en las cargas articulares, lo cual puede desencadenar dolor patelofemoral. El estudio tuvo como objetivo, correlacionar la alteración de la cinemática del pedaleo con el dolor patelofemoral en ciclistas aficionados del Team The Door´s. El estudio es de tipo correlacional con un enfoque cuantitativo, prospectivo y transversal, se realizó con 30 ciclistas hombres, que tuvieron una medida de edad de 38,97 ± 13,54 años. Para la evaluación de la cinemática del pedaleo, se utilizó el software Kinovea con análisis 2D, obteniendo rangos articulares de tobillo, rodilla y cadera. En la evaluación del dolor patelofemoral utilizó el cuestionario Kujala (AKPS). Los datos se analizaron estadísticamente de manera inferencial y descriptiva, utilizando correlación de Pearson y prueba de T Student, con un nivel de significancia de p < 0,05. Los resultados mostraron un rango articular normal en rodilla, rango de tobillo con una reducción del (19,3° ± 6,0) y en cadera se registró compensaciones con un rango amplio de (117,3 ° ± 7,7). El 100% de participantes presentó dolor patelofemoral, especialmente al subir escaleras o mantener rodillas flexionadas. En las variables cinemáticas y el puntaje Kujala, no se encontraron correlaciones estadísticamente significativas (r entre -0,23 y -0,017; p >0,05) por lo que se identifica la ausencia de una relación directa. En conclusión, no se encontró una relación significativa entre el dolor patelofemoral y la cinemática del pedaleo.
