“Glomus carotídeo en paciente con antecedente de microadenoma hipofisario en tratamiento con cabergolina” ¿Existe relación?

Abstract
INTRODUCTION: Carotid paragangliomas originating from the carotid body or
chemodectomas; they are generally rare, commonly benign tumors, accounting
for 0.5% of all body tumors, but they comprise the majority of head and neck
paragangliomas. They occur in patients residing in the Sierra region, our patient
has a particular case taking into account the pathologies developed in her life,
the pharmacological treatment established and the different places where she
lived.
OBJECTIVES: To identify the evolution and development of the carotid glomus
tumor in the patient.
Describe the diagnostic and therapeutic procedures used during patient
management.
Relate the prolactin-secreting pituitary microadenoma and the development of
the carotid glomus. CASE PRESENTATION: Female patient, 45 years old. With a personal history
of hypothyroidism and prolactin-producing pituitary microadenoma in treatment
with cabergoline. She went to the health home for presenting sporadic dysphonia
for 2 years of evolution; the discomfort is accompanied by a sensation of a palpable, non-painful mass in the right neck region. Physician values patient and
requires complementary exams for better management. In a new consultation
with a specialist, the diagnosis is ratified and surgical conduct is planned.
CONCLUSIONS: The development and evolution of carotid glomus tumor has
been verified in patients residing at remarkable heights above sea level. The
current treatment is surgical resection. It has not been possible to establish a
scientific support, much less retrospective studies, of prolactin-secreting pituitary
microadenoma, cabergoline, and carotid glomus tumor.
RECOMMENDATIONS: Post-surgical medical follow-up is recommended, they
may present complications. The review of the clinical case is recommended since
it can open doors of investigation in patients with a similar pathological picture.

INTRODUCCIÓN: Los paragangliomas carotideos que se originan del cuerpo
carotideo o quimiodectomas; generalmente son tumores raros comúnmente
benignos, llegando a ser el 0.5% de todos los tumores corporales, pero
comprenden la mayoría de los paragangliomas de cabeza y cuello. Se presentan
en pacientes residentes en región sierra, nuestra paciente tiene un caso en
particular tomando en cuenta las patologías desarrolladas en su vida, tratamiento
farmacológico instaurado y a los diferentes lugares donde vivió.
OBJETIVOS: Identificar la evolución y desarrollo del tumor de glomus carotideo
en el paciente.
Describir los procedimientos diagnósticos y terapéuticos utilizados durante el
manejo del paciente.
Relacionar el microadenoma hipofisario secretor de prolactina y el desarrollo del
glomus carotideo. PRESENTACIÓN DE CASO: Paciente femenina, 45 años de edad. Con
antecedentes personales de hipotiroidismo y de microadenoma hipofisario
productor de prolactina en tratamiento con cabergolina. Acude a casa de salud
por presentar disfonía esporádica desde hace 2 años de evolución, la molestia
se acompaña con sensación de masa palpable no dolorosa en región lateral deL
cuello derecho. Médico valora a paciente y requiere de exámenes
complementarios para un mejor manejo. En una nueva consulta a especialista
se ratifica diagnóstico y se planifica conducta quirúrgica. CONCLUSIONES: El desarrollo y evolución de tumor de glomus carotideo se ha
comprobado en pacientes que residen en alturas notables sobre el nivel del mar.
El tratamiento actual es la resección quirúrgica. No se ha logrado relacionar
mediante un sustento científico ni mucho menos estudios retrospectivos al
microadenoma hipofisario secretor de prolactina, a la cabergolina y al tumor de
glomus carotídeo.
RECOMENDACIONES: Se recomienda seguimiento médico postquirúrgico,
pueden presentar complicaciones. Se recomienda la revisión del caso clínico ya
que puede abrir puertas de investigación en pacientes con parecido cuadro
patológico.
