Abstract
Objective: Characterize patients with cholecystogastric fistulas and identify the possible post-
surgical complications that they may present.
Materials and methods: Retrospective descriptive study of clinical case analysis.
Discussion: Bilioenteric fistulas are an entity that can be defined as abnormal communication
between the biliary system and the gastrointestinal tract and that can occur spontaneously or as
in most cases due to a complication of untreated gallstones. Where the main objective of the
treatment for this pathology is to achieve the decompression of the bile duct and prevent
recurrences, with an expectant and conservative management of the originated fistula. since it
can cause diverse clinical consequences that in some situations endanger the life of the patient.
We present the case of a 55-year-old patient with no significant history who came to the
emergency department of the Ambato regional hospital with an acute abdomen and who,
through imaging and Tokyo criteria, has reached a diagnosis of cholecystitis, for which a
decides to perform emergency laparoscopic cholecystectomy in which a cholecystogastric
fistula is found incidentally with opening of the stomach towards the vesicular plastron and
gastric wall necrosis in addition to subhepatic and perigastric abscesses, for which it is decided

to perform debridement of the multiple adhesions, raffia of stomach in three planes, subtotal
cholecystectomy and cholecystostomy, she was transferred to the ICU for 14 days after which
she was discharged and followed up. Eight months after cholecystectomy, the patient presents
a torpid clinical picture triggered by sepsis of biliary origin, for which the patient is admitted
to the intensive care service with follow-up by general surgery, due to the complex resolution
of her clinical picture and the need for a A surgical team experienced in hepatobiliary surgery
activates the network and is referred to a tertiary center in this case to the Eugenio Espejo
Specialties Hospital for biliary reconstruction and performance of Hepatojejunostomy where
she was operated electively, thus resolving her pathological picture.
Conclusion: It was determined that cholecystogastric fistulas are not so frequent clinical
entities with reported incidences of less than 1.2 and 3% of biliary pathologies in hepatobiliary
surgeries; being a medical challenge since the vast majority are clinically silent or present
vague digestive symptoms, and there are no diagnostic modalities specific enough to make a
preoperative diagnosis, so doctors should be aware of its possibility in patients with
cholelithiasis of long duration and chronic cholecystitis. Once the fistula is detected
intraoperatively, it is imperative to perform a cholecystectomy, cholecystostomy or
percutaneous drainage, which allows decompression of the gallbladder and later, in a second
stage, consider the repair of the existing fistula, if it warrants it or in patients with high surgical
morbidity and mortality and poor response to conservative treatment.

Objetivo: Caracterizar a los pacientes que presentan fístulas colecistogástricas e identificar las
posibles complicaciones post quirúrgicas que estos puedan presentar.
Materiales y métodos: Estudio descriptivo retrospectivo de análisis de caso clínico.
Discusión: Las fístulas bilioentéricas son una entidad que se puede definir como la
comunicación anormal entre el sistema biliar y el tracto gastrointestinal y que pueden ocurrir
de manera espontánea o como en la mayoría de los casos debido a una complicación de litiasis
vesicular no tratada. En donde el objetivo principal del tratamiento para esta patologìa es lograr
la descompresión de la vía biliar y prevenir recurrencias,con un manejo expectante y
conservadora de la fistula originada. ya que puede provocar consecuencias clínicas diversas
que en algunas situaciones ponen en peligro la vida del paciente. Se presenta el caso de una
paciente de 55 años sin antecedentes de importancia que acude al servicio de urgencias del
hospital regional de Ambato con cuadro de abdomen agudo y que mediante imagen y criterios
de Tokio se ha llegado a un diagnóstico de colecistitis por lo cual se decide realizar
colecistectomía laparoscópica de emergencia en la cual se halla de forma incidental una fístula
colecistogástrica con apertura de estómago hacia plastrón vesicular y necrosis de pared gástrica
además de abscesos subhepático y perigástrico, por lo que se decide realizar desbridamiento
de las múltiples adherencias , rafia de estómago en tres planos, colecistectomía subtotal y
colecistostomía, es trasladada a UCI por 14 días luego de lo cual se decide su alta y se hace
seguimiento. Ocho meses post colecistectomía la paciente presenta una clínica tórpida
desencadenada por una sepsis de origen biliar, por lo que paciente es internada en el servicio
de terapia intensiva con seguimiento por cirugía general , debido a la complejidad resolutiva
de su cuadro clínico y la necesidad de un equipo quirúrgico experimentado en cirugía
hepatobiliar se activa red y se deriva a un centro terciario en este caso al Hospital de
Especialidades Eugenio Espejo para una reconstrucción biliar y realización de
Hepatoyeyunostomìa donde fue intervenida de manera electiva resolviendo así su cuadro
patológico.
Conclusión: Se determino que las fístulas colecistogástricas son entidades clínicas no tan
frecuentes con incidencias reportadas menores al 1.2 y 3% de las patologías biliares en
cirugías hepatobiliares; siendo un reto médico ya que en su gran mayoría son silenciosas
clínicamente o presentan síntomas digestivos vagos, y no existen modalidades de diagnóstico
lo suficientemente específicas como para hacer un diagnóstico preoperatorio por lo que los
médicos deben ser conscientes de su posibilidad en pacientes con colelitiasis de larga duración
y colecistitis crónica.Una vez que se detecta la fístula intraoperatoriamente, es imperativo la
realización de una colecistectomía,colecistostomía o drenaje percutáneo , que permita la
descompresión de la vesícula y posteriormente, en un segundo tiempo, considerar la reparación
de la fístula existente, si ésta lo amerita o en pacientes con elevada morbimortalidad quirúrgica
y escasa respuesta al tratamiento conservador.
