Título : El Presupuesto General del Estado asignado al sector salud y su influencia sobre la tasa de cobertura de atención al usuario. Caso IESS Ambato

Resumen : El escenario actual provocado por la pandemia del Covid – 19, sin duda ha tenido efectos negativos no solo para el ámbito de la salud, sino también para toda la estructura de los sistemas productivos, económicos y financieros. En virtud de los efectos adversos provocados en el sistema de salud, resulta necesario determinar si de alguna manera el Presupuesto General del Estado asignado al sector salud incide sobre la tasa de cobertura de atención al usuario. Para lo cual se identificó y se seleccionó una institución muy representativa de la zona 3 del Ecuador, como lo es el Hospital General Ambato perteneciente al Instituto Ecuatoriano de Seguro Social. La información de fuentes primarias fue obtenida de los principales reportes del sistema financiero eSIGEF, así como las estadísticas de atención y gestión hospitalaria del periodo en el cual se inició la pandemia. En la contextualización, se describieron los principales efectos provocados por el virus Covid-19 a nivel nacional, provincial y local, así como también se realiza la descripción del problema de investigación, particularmente a un caso específico, lo cual se ha establecido para este análisis en particular al Hospital General Ambato. Los objetivos delineados se enfocan en entender de mejor manera el contexto en el cual se encuentra la problemática, así como las hipótesis a contrastarse a través del análisis econométrico de cada uno de los modelos. La metodología aplicada es de carácter cuantitativo con enfoque descriptivo – correlacional, mediante la técnica de mínimos cuadrados se obtuvo que a un nivel de confianza del 90% la variable independiente (presupuesto codificado) incide sobre la variable dependiente (tasa de cobertura). Se ha planteado un modelo de regresión lineal de la variable independiente: Asignación presupuestaria sobre la variable dependiente: Tasa de cobertura de atención a los usuarios; caso Hospital General de Ambato. Con un elevado margen de significancia estadística con un nivel de confianza del 95%. Además, se realizó el pronóstico para cada una de las variables mediante el uso de los modelos VAR y ARIMA. Los cuales si muestran diferencias considerables en sus predicciones. Finalmente se concluye que existe una tendencia decreciente sobre la asignación de recursos por parte del Estado a este sector, mientras que la tasa de cobertura seguirá indicando el 100% de su capacidad para atender la emergencia sanitaria. Para culminar es importante destacar que, dada la situación actual, las entidades públicas deben enfocar sus esfuerzos a través de sus servicios de salud eficientes, esto sin duda fortalecerán el logro de uno de los ODS (Objetivos de desarrollo sostenible) dentro de los cuales garantizar la salud de la población es sinónimo y reflejo de un buen gobierno y que lastimosamente ese término se desvincula fuertemente de la situación actual del país. Se brindan una serie de recomendaciones sobre todo y especialmente dirigido a que las autoridades públicas cuenten con un instrumento adicional de apoyo que les permita, basándose en la evidencia empírica – matemática, tomar decisiones para mejorar la utilización de los recursos financieros destinados al sector salud.
Descripción : The current scenario caused by the Covid-19 pandemic has undoubtedly had negative effects not only for the health field, but also for the entire structure of the productive, economic and financial systems. By virtue of the adverse effects caused in the health system, it is necessary to determine if in any way the General State Budget assigned to the health sector affects the coverage rate. For which a very representative institution of zone 3 of Ecuador was identified and selected, such as the Ambato General Hospital which belongs to the Ecuadorian Social Security Institute. The information of primary sources was obtained of the principal reports of the financial system eSIGEF, as well as the statistics of the attention and hospital management of the period from the begging of the pandemic. On contextualization, the main effects caused by the Covid19 virus were described at the national, provincial and local levels, as well as a description of the research problem, particularly to a specific case, which has been established in particular to the Ambato General Hospital. The outlined objectives focus on better understanding the context in which the problem is found, as well as the hypotheses to be tested through the econometric analysis of each of the models. The applied methodology is quantitative with a descriptive-correlational approach, using the least squares technique it was obtained that at a confidence level of 90% the independent variable (codified budget) affects the dependent variable (coverage rate). A linear regression model of the independent variable has been proposed: Budget allocation on the dependent variable: User service coverage rate; Ambato General Hospital case. With a high margin of statistical significance with a confidence level of 95%. In addition, the forecast was made for each of the variables by using the VAR and ARIMA models. Which if they show considerable differences in their predictions. Finally, it is concluded that there is a decreasing trend regarding the allocation of resources by the State to this sector, while the coverage rate will continue to indicate 100% of its capacity to attend the health emergency. To conclude, it is important to note that, given the current situation, public entities must focus their efforts through their efficient health services, this will undoubtedly strengthen the achievement of one of the SDGs (Sustainable Development Goals) within which to guarantee the health of the population is synonymous with and a reflection of good government and that unfortunately this term is strongly disconnected from the current situation in the country. A series of recommendations are given above all and especially aimed at providing public authorities with an additional support instrument that allows them, based on empirical - mathematical evidence, to make decisions to improve the use of financial resources destined for the health sector.
