    Complex cysts of adnexal origin are more common in reproductive age (20-45 years); May be of ovarian origin or of the fallopian tube, and may also correspond to non-gynecological pathology. Ovarian cysts are the most frequent, filled with a solid, liquid, or mixed substance. "Complex" cysts, as the name implies, are more complicated than other types. They can be cancerous and even deadly, if left untreated. Identification of the exact cause of the complex ovarian cyst is not possible. A laparoscopy is required for your treatment. We report the case of a 24-year-old female patient, who visits the "El Dorado Health Center" for routine check-ups and during the physical examination, a mass at the hypogastrium level is evident, the patient reports that she has not noticed dough. Therefore, during the accomplishment of the clinical history it is collected information about the evolution, signs and symptoms that the patient presented to near that mass. Therefore it is decided to perform complementary examinations such as abdominal ultrasound that reported pelvic complex cyst, probably of adnexial origin, also performs Computerized Axial Tomography which reports cystadenoma. The laboratory findings, showed high Ca 125 levels, questioning the diagnosis, which led to a diagnosis of laparotomy, finding grade IV endometriosis. Postoperative evolution was satisfactory.
