dc.descriptionLas Enfermedades Hipertensivas del Embarazo son una causa importante de morbilidad y mortalidad materna y fetal con una incidencia variable según la población estudiada, estimándose que en los países en vías de desarrollo es aproximadamente del 5 - 10%, siendo más frecuente en los extremos de la edad reproductiva. Cada 3 minutos muere una mujer en el mundo debido a la Preeclampsia, siendo la principal causa de muerte materna en el mundo. Se presenta Caso Clínico sobre una paciente de 43 años de edad con Antecedentes Patológicos Personales de Hipotiroidismo, quien acude a consulta externa donde se ingresa por presentar al examen físico Tensión Arterial se encuentra dentro de 143/86 mmHg, abdomen útero Gestante, Producto Único, AFU acorde a edad gestacional, Presentación Cefálica, Dorso Izquierdo, FCF 152 lpm, Útero Irritable, actividad uterina de 2-3 contracciones en 10 minutos, región Inguino genital: Genitales de Nulípara, no presencia de sangrado; en extremidades se encuentra edema de miembros inferiores ++/+++. Los Exámenes Complementarios al Ingreso reportan Leucocitosis acompañada de Neutrofilia, con Hemoglobina y Hematocrito dentro de parámetros normales. Por lo que paciente es ingresada en el Servicio de Obstetricia y Ginecología en el Hospital IESS Ambato, con diagnóstico de Embarazo de 34.4 semanas y Trastorno Hipertensivo del Embarazo. En su tercer día de Hospitalización paciente refiere aumentar Edema de Miembros Inferiores. Presión Arterial de 140/80 mmHg, facies edematosas +/+++, y en extremidades edema de miembros inferiores bilateral +++/+++. Se realiza Microalbuminuria Es sometida a Cesárea Segmentaria por cuadro de Preeclampsia, presentando en el transoperatorio Atonía Uterina y Síndrome Adherencial. Según el Parte Operatorio los Hallazgos fueron pared fibrótica, líquido en cavidad de aproximadamente 100 cc, útero gestante, líquido amniótico claro sin grumos, recién nacido vivo femenino con APGAR y peso adecuado, placenta anterior acreta y sangrado de 1000 cc; posterior intervención se solicita interconsulta a Cirugía General por causa de sangrado continuo y shock hipovolémico realizando Histerectomía Obstétrica y Laparotomía Exploratoria; finalmente ingresa a la Unidad de Cuidados Intensivos del Hospital IESS Ambato con diagnóstico de Shock Hipovolémico, Posthisterectomía Obstétrica por Atonía Uterina, Sangrado de 3er período de Parto, Adeherensiolisis por Endometriosis grado IV y Preeclampsia, evoluciona favorablemente, y es dada de alta el día 17 de Febrero del 2017.es_ES

On her third day of patient hospitalization she referred to increase lower limb edema. Arterial pressure 140/80 mmHg, edematous facies + / +++, and in extremities bilaterally bilateral edema +++ / +++. Microalbuminuria and Proteinuria in 24-hour urine are performed which are positive. It is submitted to Cesarean Section by Preeclampsia, presenting in the transoperative Atonia Uterine and Adherence Syndrome. According to the Operative Part the findings were fibrotic wall, fluid in cavity of approximately 100 cc, pregnant uterus, clear amniotic fluid without lumps, female live newborn with APGAR and adequate weight, placenta accreta anterior and bleeding of 1000 cc; Posterior intervention is requested interconsultation to General Surgery because of continuous bleeding and hypovolemic shock performing Obstetric Hysterectomy and Exploratory Laparotomy; Finally enters the Intensive Care Unit of the Hospital IESS Ambato with diagnosis of Hypovolemic Shock, Obstetric Posthisterectomy for Uterine Hypotonia, Bleeding of 3rd period of Delivery, Adeherensiolisis by endometriosis grade IV and Preeclampsia, evolves favorably, and is discharged on day 17 Of February 2017.es_ES
