Abstract: Cardioembolic ischemic stroke is caused by blockage of a brain artery by embolic material from the heart. To talk about of a cardioembolic mechanism that causes a stroke is necessary to demonstrate the presence of a structural alteration or heart rhythm that predisposes to thrombus formation inside the heart chambers. Currently about 80% of the disease Cerebrovascular is due to a cerebral ischemic infarction with 30% of cardioembolic origin, which is due to the migration of an embolus from the heart chambers or the proximal segment of the thoracic aorta. The next clinic case to analyze is from a 51 years old female with a history of hypothyroidism for 7 years and an unspecified maternal heart disease, a brother with a pacemaker for sinus node dysfunction, atrial flutter ablated and dilated cardiomyopathy, moreover sister with a pacemaker for atrial flutter slow ventricular response and her mother with CVD (deceased). The patient presents symptoms for the first time in December 2013 with dyspnea of small efforts and sudden appearance accompanied by oppressive type chest pain for 2 minute eva5 / 10 without irradiation, without palpitation or syncope, the symptoms disappear spontaneously. In February 2014 she presents picture characterized by chest pain accompanied 5/10 oppressive type without irradiation asthenia, no dizziness, syncope or palpitations 5 minutes long that subsides spontaneously , the same picture is repeated every 3 days, 2 times a day. The patient goes to doctor who diagnosed her with cardiac arrhythmia and is transferred to HCAM to study her pathology. On May 23, 2014 the patient goes to the hemodynamics of HCAM for checking rhythm disturbance. The patient is diagnosed with typical atrial flutter with subsequent plan ablation and then is evaluated about sinus node function, for which requested laboratory tests before initiation of anticoagulation, which was within normal parameters, and subsequent initiation of anticoagulation with Dabigatran 150 mg BID to June 21. The ablation is scheduled for May 24, 2014 prior evaluation by Anesthesiology cataloging with ASA III and enters the operating room hemodynamics where they perform ablation of Isthmus Cavotricuspide, at 24 hours post-ablation of typical atrial flutter. The 2015/05/06 patient was admitted to the Emergency Hospital Latacunga IESS, relatives reported that in the afternoon without apparent cause aphasia and deterioration present level of consciousness it was immediately taken to health center Sigchos and later referred to this hospital where the patient was aphasic, did not respond to verbal commands, responds to painful commands, performs spontaneous movements SV: TA: 170/90, Sat O2: 88% 2 liters Fc: 61 to EF: disoriented, agitated patient sleepy, Glasgow 10/15, positive Babinski, bilateral pulmonary rales scattered, heart sounds bradycardic heart; was diagnosed with Ischemic CVD, more grade III AV block due to lack of availability of tests and the status of the family patient requested voluntary discharge and transfer to private clinic (Clinical PASTEUR); the date 2015/05/06 the patient is transferred to HCAM and entered the service of cardiology and post neurology at diagnosing CVD Ischemic (cardioembolic origin in the territory of MCA Middle Cerebral Artery) total more BAV that remains sequel Broca right hemiplegia same as in rehabilitation and anticoagulated with Warfarin 5 mg ¼ of tablet and monthly checks for service Hemodynamics HCAM
Description: El ictus isquémico cardioembólico se debe a la obstrucción de una arteria cerebral por material embólico procedente del corazón. Para poder hablar de un mecanismo cardioembólico como causante de un ictus es necesario demostrar la presencia de una alteración cardiaca estructural o del ritmo que predisponga a la formación de trombos en el interior de las cavidades cardiacas. En la actualidad Aproximadamente el 80% de la Enfermedad Cerebro Vascular se debe a un infarto Isquémico cerebral con un 30 % de origen cardioembólico, el cual se debe a la migración de un embolo proveniente de las cavidades cardíacas o del segmento proximal de la aorta torácica. El siguiente caso a analizar se trata de una paciente de 51 años de edad, sexo femenino con antecedentes de hipotiroidismo hace 7 años y de línea materna enfermedad cardiaca que no especifica, paciente presenta por primera vez en diciembre de 2013 disnea de pequeños esfuerzos y de aparición súbita acompañado de dolor torácico tipo opresivo de 2 minutos de duración Eva5/10 sin irradiación sin palpitación ni sincope, cuadro que desaparece espontáneamente, en el mes de febrero 2014 presenta cuadro caracterizado por dolor retroesternal Eva 5/10 tipo opresivo sin irradiación acompañada de astenia, no mareo o sincope o palpitaciones de 5 minutos de duración que cede espontáneamente, mismo cuadro que se ha repetido cada 3 días por 2 veces al día, la misma acude a facultativo quien diagnostica de arritmia cardiaca y transfieren a Hospital Carlos Andrade Marín (HCAM) para estudio de patología, el 23 de mayo del 2014 acude al servicio de hemodinámica del HCAM y diagnostican de Flutter auricular típico con posterior plan de ablación y luego evaluación de función sinusal, y posterior inicio de anticoagulación con Dabigatran 150 mg BID hasta el 21 de junio. El 2015/05/06 la paciente acude al Servicio de Emergencia del Hospital IESS Latacunga, familiares refirieron que en horas de la tarde y sin causa aparente presento afasia y deterioro del nivel de conciencia la misma que fue inmediatamente llevada a centro de salud de Sigchos y posterior referida a esta casa de salud, al momento de la valoración la paciente se encontraba afásica, no respondía a comandos verbales, responde a comandos dolorosos, realiza movimientos espontáneos SV: TA: 170/90, Sat O2: 88% A 2 Litros FC: 61 al EF: paciente desorientada, somnolienta agitada, Glasgow 10/15, Babinski positivo, pulmonar estertores bilaterales diseminados, corazón ruidos cardiacos bradicardicos; se diagnosticó de ECV Isquémico, mas bloqueo AV grado III, por falta de disponibilidad de exámenes y el estado de la paciente familiares solicitaron el alta voluntaria y el traslado a clínica particular(Clínica PASTEUR); el dia2015/05/06 la paciente es transferida al HCAM e ingresada al servicio de cardiología y posterior a neurología en el que diagnostica ECV Isquémico (origen cardioembolico en territorio de ACM Arteria cerebral media) mas BAV total del que mantiene como secuela afasia de broca, hemiplejia derecha mismas que se encuentra en rehabilitación y anticoagulada con Warfarina 5 mg ¼ de tableta y en controles mensuales por el servicio de Hemodinámica del HCAM.
