    FIBRILACIÓN AURICULAR Y ENFERMEDAD RENAL CRÓNICA: INTERACCIÓN Y RELEVANCIA CLÍNICA

    Chronic kidney disease (CKD) is characterized by prolonged alterations in renal structure or function, with severe implications such as hypertension, cardiac and neurological diseases. Atrial fibrillation (AF), a cardiac arrhythmia, shows a complex relationship with CKD: the presence of CKD increases the risk of developing AF, and vice versa. Several factors, such as inflammation, the renin-angiotensin-aldosterone system, anemia, and electrolyte disturbances contribute to this relationship. Both conditions also increase the risk of stroke and mortality. Proper management involves considering the complexity of these connections to reduce the associated risks and improve patient prognosis.

    Relato de vivencias de un paciente con insuficiencia renal cronica en base al modelo de callista Roy

    A adaptação de pacientes com Insuficiência Renal Crônica (IRC) submetidos a procedimentos invasivos exige uma série de alterações físicas e psicológicas muito significativas. Objetivo: Relatar as experiências de um paciente com Insuficiência Renal Crônica com base no modelo de Callista Roy. Método: Pesquisa descritiva com abordagem qualitativa, do tipo fenomenológico exploratório. Foram analisadas as vivências de um paciente com Insuficiência Renal Crônica em tratamento dialítico, escolhido de acordo com os seguintes critérios de inclusão: maior de idade e familiar. As informações foram coletadas por meio de entrevista semiestruturada baseada no modelo de Callista Roy, explorando o processo de adaptação, função fisiológica, autoconceito e função do papel de cuidador presentes nas experiências relatadas pelo mesmo paciente. Resultados: As informações obtidas mostram que a adaptação de uma doença envolve diversos aspectos sociais, fisiológicos, psicológicos e culturais, desencadeando níveis de estresse, ansiedade e depressão pelo simples fato de seu estilo de vida gerar um impacto negativo, razão pela qual busca melhorar a qualidade de vida de cada paciente que tem uma doença crônica e recebe tratamento por toda a vida, adaptando-se assim às novas mudanças. Conclusões: Existe capacidade de adaptação e enfrentamento em pacientes com Insuficiência Renal Crônica e que recebem tratamento dialítico, mostrando tendências estáveis no uso de estratégias para melhorar a qualidade de vida

    Percepciòn de los pacientes con insuficiencia renal crónica sobre los cuidados proporcionados por el personal de enfermería del centro de hemodiálisis Dialvida Ambato

    The present research refers to the perception of patients with chronic renal failure about the care provided by the nursing staff of the Dialvida Ambato Hemodialysis center. The objective was to describe the perception of patients with chronic renal failure regarding the care provided by the nursing staff of the Dialvida Ambato Hemodialysis Center. For this, a qualitative, descriptive, cross-sectional, field and prospective study was carried out to find out the ideas, thoughts and opinions of patients with chronic renal failure who undergo hemodialysis on nursing care. Due to its effect, the study technique that was carried out was the semi-structured interview with a total of 14 patients, fulfilling the information saturation criterion. The results in the following research were phenomenological, conceptual and descriptive of the perception of nursing care, humanized care and the influence of the disease process in daily life. The conclusions that were obtained from the research were positive and of satisfaction regarding nursing care and humanized care, but negative responses were found such as lack of acceptance and adaptation to their change in the patients' lifestyle. Finally, it is recommended that the care already established by the nursing staff continue to be applied and the humanized care should continue and be reinforced among the patients and the multidisciplinary team, it should also inquire about the influence on the quality of life in relation to the treatment and diagnosis.

    “Influencia de la autoestima en las estrategias de afrontamiento que adoptan los pacientes que padecen insuficiencia renal crónica en el servicio de medicina interna del H.P.D.A. en el periodo enero – marzo 2012”.

    Patients with Renal Chronic Failure adopted different coping strategies to fight against his illness, such strategies can be influenced by several causes, among which stands out the level of self-esteem. There are some coping strategies such as active coping and focused on the problem appear to be associated with positive psychological results, such as a better adaptation to the new status of sick; in Chronic Renal Failure While coping by avoidance, confrontation, of distance, of self-control and escape appears to be associated with negative psychological outcomes, such as depression and anxiety. In the period between January - March 2012 were investigated 22 patients with Renal Chronic Failure were entered in the service of medicine internal of the Ambato Hospital, of which 16 patients had a low level of self-esteem, 5 patients a normal level and 1 patient a high level of self-esteem. Within the more coping strategy for patients with chronic renal failure was the "disengagement behavioral" with a percentage of 9.2, then "Acceptance" with 9, the "cognitive disengagement" with 8.7, suggesting strategies that do not help to lead to disease and the least used were "Planning" with 7.1, "Positive reinterpretation" and the "focus and release and release emotions" with 6.4 and finally the "active coping" with a 5.9. So it shows that the low self-esteem level influences in a negative way at the time of adopting a strategy of coping that is not adequate to combat the disease.

    This research work was to question the known modifiable factors that influence the progression of the stages of chronic renal failure Menydial Clinic of the Kidneys in the city of Riobamba, the research that underlies the present work is the development of a prevention guide to reducing the modifiable factors that influence the progression of stages. The decision to create a prevention guide is due to the need to improve the quality of life of patients with chronic renal failure and slow the progression of the stages of the disease, which is based on diet, exercise, behavioral therapies, behavioral, control fluid intake. The design of this research will be framed in the form of documentary and field research, because they determine the modifiable factors that influence the stages of the patients treated at the Clinic Menydial of Kidneys in the city of Riobamba in 2011. Through surveys and review of medical records for patients 31 to 65 years of age. The research is descriptive, retrospective, because it seeks to investigate the reality on the research problem, to design a guide preventive and alternative solutions. The focus of the study was in qualitative terms based on a population composed of 100 patients diagnosed with chronic renal failure, we excluded patients younger than 31 years and over 65 years, thus leaving a total of 43 patients with renal insufficiency Chronic between ages 31 to 65 years for the study. Reaching a conclusion after analyzing the results, one can say that patients with hypertension are 2.54 times more risk of progression in stage of their disease than patients who have diabetes, so it can be determined blood pressure and diabetes influence the evolution of the stage of renal failure patients a more prevalent than another.
