    “Determinación de cistatina c e identificación de la relación con la creatinina sérica en el diagnóstico de insuficiencia renal en pacientes con diabetes mellitus tipo 2”.

    The present research project has as main objective to determine the Cystatin C and to identify the existing relation with the serum Creatinine to diagnose Renal Insufficiency in patients with Type 2 Diabetes Mellitus. The study was performed with the participation of 30 patients who attend the Diabetic Club of the Pelileo Basic Hospital. Glucose levels were determined by observing that 73% of the population presented hyperglycemia and blood was drawn to determine serum creatinine and Cystatin C, and GFR was estimated by applying two fórmulas (MDRD-4 and Larson), observing that estimated GFR from Creatinine was decreased in 100% of the population while GFR evaluated from Cystatin C was found to be normal in 50%. With estimated GFR, the patients were grouped according to the stage of renal disease in which they were found. In this way, it was observed that serum Creatinine categorized patients into only three stages: moderate, severe and terminal renal damage; while with Cystatin C it was possible to categorize and effectively differentiate disease stages by categorizing the 5 disease stages (normal, mild, moderate, severe and advanced). By means of the Chi-square test, we can verify that the variables have a positive correlation, so we accept the alternative hypothesis that mentions "if there is a relationship between serum Creatinine and Cystatin C for the diagnosis of renal failure", therefore The determination of Cystatin C is a great help in measuring GFR and determining the stage of renal damage.

    “Intervención de enfermería en paciente adulto mayor con enfermedad renal crónica secundaria a diabetes.”

    The present clinical case study aims to determine the care to be taken by the patient with Chronic Renal Failure secondary to Diabetes. In this study, a 78-year-old male patient, a married civil status, with a history of Type 2 Diabetes Mellitus for 20 years, with treatment and Arterial Hypertension, was admitted to the Ecuadorian Social Security Institute (IESS). Time being an older adult patient, failure to comply with the prescribed medical indications, and lack of care within the home, all of these are the main factors that triggered chronic renal failure in the patient. Because of the seriousness of these, began pharmacological treatment and about 10 months ago began with dialysis. The importance of dialysis for the health of the patient is communicated to family members (wife and daughter), but there is no favorable response. Expectant behavior. The Methodology in this case was done through analysis, bibliographical and documentary research, description, the application of surveys and observation, which allows us to establish the patient's condition and the nursing care that can be performed. The results found were both physical and emotional difficulties which altered their lifestyle, coming from their illness, also lack of family care. In Conclusion we identified the main factors that triggered the patient's problems, this information was compiled to design a nursing care guide aimed at the patient and family to improve their quality of life.

    “Determinación del ancho de la curva de distribución Eritrocitaria en pacientes con dmt2 y su relación con el daño renal”

    The present research project aimed to determine the Width of the Erythrocyte Distribution Curve (RDW) in patients with type 2 diabetes mellitus and its relation to renal damage. The RDW is an index to evaluate the variation of red blood cell volume. Recently, RDW was significantly associated with increased mortality in patients with Diabetes Mellitus, changes in RDW are also associated with the production of some inflammatory cytokines, and oxidative stress play an important role in the pathogenesis of damage renal. Regarding the methodology, the work approach was qualitative, applying field research, laboratory, and a level of longitudinal analytical research. We included 38 patients with DMT2 with a mean age of 60.39 ± 14.93 years of age. Each patient underwent two blood biometric and blood chemistry analyzes and correlated the values obtained. Based on the results of calculating the estimated glomerular filtration rate (eGFR) in the study population, it was possible to divide into 5 categories: 12 patients with normal or elevated glomerular filtration rate (GFR), 9 with Damage Renal and mild decrease in GFR, 1 with mild-moderate GFR, 2 with moderate GFR, and 14 with renal failure. Multiple logistic regression indicated that RDW (p <0.001) was an independent risk factor for kidney damage in mellitus diabetes type 2.

    Several studies show a clear association between diabetes mellitus and the development of pancreas adenocarcinoma because its complications are not uncommon and they have to be treated in the best way possible keeping a metabolic equilibrium. This study case has to do with an elderly male patient with a pancreatic cancer associated to diabetes due to the fact that this gland produces juices that help assimilate food and the hormones that propitiate the regulation of glucose levels in the blood. Most of the information was gathered from the clinical history of the patient. This information, which was necessary for the analysis, was collected with the patient’s and the family’s cooperation and was used as a reference point. The nursing assistance is based on the scientific method which allow us to provide care in a rational, logical, and systematic manner. This allows us as future nursing professionals make independent decisions in order to achieve objectives deriving from our assistance which will improve the patient’s health state. Each day, the impact of the assistance to diabetic peopleis more noticeable, as well as the evidence that education is essential on its therapeutic approach. That’s why, with this case, we attempt to increase the level of trust on the daily work of the nursing professional. This will turn into a more efficient service for people with this chronic condition.
