Abstract: In the following clinical case, the presence of neuropathic pain as a result of a sacral-iliac and acetabular fracture was analyzed in a 39-year-old patient; the patient after presenting unbearable pain, is operated on after twenty days, presenting a functional limitation of the lower right limb and diagnosed as lumbo-sacral plexus axonotmesis. It was developed under a qualitative approach, the research is of phenomenological type, the primary information was obtained through the application of scripts of interviews to the wife and the physiotherapist, a physical and sensitive physiotherapeutic evaluation card; where muscle strength, joint width, muscle mass volume, superficial and deep sensitivity, as well as root maneuvers were measured, in addition, the respective informed consent is executed. The secondary information was obtained through the analysis of the patient's medical records of both the IESS Ambato General Hospital and the Specialties Hospital of the FFAA N° 1; In addition to the review 31 scientific articles collected from the PubMed, Medline, Scielo, LILACS, RedAlyC, PEDro, and Google Scholar databases. In the patient, it was determined that there is currently a presence of moderate pain and a decrease in strength, joint width and considerable muscular mass of the right lower limb; while in the sensitivity, a compromise of the branches of L4, L5, S1 and S2 of the lumbosacral plexus was detected. So the proposed treatment plan is based on the functional recovery of the right lower limb.
Description: En el siguiente caso clínico, se analizó la presencia de dolor neuropático a consecuencia de una fractura sacro-iliaca y acetabular, en un paciente de 39 años de edad; el paciente luego de presentar dolor insoportable, es intervenido quirúrgicamente a los 20 días, presentando una limitación funcional de miembro inferior derecho y diagnosticado como axonotmesis de plexo lumbo-sacro. Se desarrolló bajo un enfoque cualitativo, la investigación es de tipo fenomenológico, la información de manera primaria se obtuvo a través de la aplicación de guiones de entrevistas a la esposa y el fisioterapeuta, una ficha de evaluación fisioterapéutica motora y sensitiva; donde se midieron la fuerza muscular, la amplitud articular, el volumen de masa muscular, la sensibilidad superficial y profunda además de la realización de maniobras radiculares, además se ejecuta el respectivo consentimiento informado. La información secundaria se obtuvo a través del análisis de las historias clínicas del paciente tanto del Hospital General del IESS Ambato, como del Hospital de Especialidades de la FFAA N°1; además de la revisión 31 artículos científicos recolectados de las bases de datos de PubMed, Medline, Scielo, LILACS, RedAlyC, PEDro, y Google Scholar. En el paciente se determinó que actualmente existe presencia de dolor moderado y una disminución de la fuerza, la amplitud articular y de la masa muscular considerable del miembro inferior derecho; mientras que en la sensibilidad, se detectó un compromiso de las ramas de L4, L5, S1 y S2 del plexo lumbosacro. Por lo que el plan de tratamiento propuesto se basa en la recuperación funcional del miembro inferior derecho.
